New Dental Patient Information Arlington Heights
Setting Your Family Up for Success at Each Appointment

Your first visit to Arlington Smiles Dental Studio is an important one, as it introduces you to our practice and our friendly staff. During the first visit, we will collect background information like medical history as well as take the time for you to get to know Dr. Seredynski. Below, you will find additional information to view before your first visit to Arlington Smiles Dental Studio. Also, to help ensure your appointment goes as smoothly as possible, we invite you to take a few minutes to explore our website to educate yourself about some of the dentistry and smile solutions we offer.
Your First Visit

Your first visit to our dental office will include being met by one of our hygienists who will take the necessary radiographs of your smile before cleaning your teeth and removing any existing plaque and tartar. Dr. Seredynski will then come in to review the information provided by our team before conducting an examination and providing an official diagnosis. He will share various images of your mouth and discuss any problem areas as well as answer any questions you might have before scheduling your next appointment.
Patient Forms

After scheduling an appointment with us by phone or on our website, you will be sent a link via text or email to complete the new patient paperwork. If you have any questions or concerns, please let us know how we can help.
Dental Insurance

Arlington Smiles Dental Studio is pleased to be in-network with all major PPO dental insurance plans. When calling to schedule an appointment, we’ll inquire about your insurance and begin identifying the most effective ways to help you save on any out-of-pocket expenses you may incur. We’ll also take care of all the necessary paperwork and claims filing, as this will help alleviate any additional stress or concerns you might have about treatment.
CareCredit Financing

Certain treatments may not be covered by your dental insurance. If the out-of-pocket costs are too high to pay upfront, you can choose from one of the many available plans through CareCredit. This third-party company offers low- and no-interest options with varying timeframes so that you can choose a solution that will make it easier on your budget.
